The Change Readiness Assessment allows the change agent, project team, and other key stakeholders to measure the effectiveness of their change management efforts. The Change Readiness Assessment is completed at the start of the project and throughout the project to help identify any gaps that might exist in the change management plan for the project.


Overall the Change Readiness Assessment helps to increase the project’s likelihood of success by ensuring that a shared need exists, that the team is working toward a common goal, and that the importance of the project is communicated appropriately to help increase awareness and acceptance.
